Why Do Your Eyelids Suddenly Look So Tired — and What Can Be Done About It?

Most of the time, tired-looking eyes come down to a handful of different things happening in the skin and structures around your eyes. Excess or loose upper eyelid skin. A weakening of the muscle that lifts your eyelid. Shifting fat and volume that changes the shape of the lower lid. Or changes in colour beneath the eyes — from pigmentation, visible blood vessels or shadowing. They don’t all have the same cause — and they don’t all need the same answer.

What's Actually Going On?

There are typically a handful of things that get the most questions or cause the most concern, and they’re not the same. Knowing which of these is happening — often more than one at once — changes what will help.

Dermatochalasis — 

Excess, loose skin on the upper eyelid, usually from decreased skin elasticity with age. The muscle underneath is working fine; there’s just more skin than there used to be, and it folds. This is the most common cause of a “hooded” look.

Ptosis — 

A drooping of the eyelid margin itself, caused by weakness in the levator muscle that lifts the lid. This is a structural, muscular issue, not a skin issue. One important distinction: ptosis can develop gradually with age, but sudden or rapidly worsening ptosis — especially with other symptoms — should be seen by a physician, since it can occasionally signal something unrelated to aging.

Under-eye bags — 

Can come from a few different things, not just one. The classic cause is a weakening of the orbital septum, the thin membrane that normally holds fat in place around the eye; as it loosens, fat shifts forward and creates a persistent bulge that doesn’t go away with sleep or cold compresses. Skin laxity can create a similar look on its own, separate from fat. And temporary puffiness from fluid retention is a distinct, usually short-lived cause, often triggered or worsened by other factors such as allergies, fatigue, or a poor night’s sleep.

Hooded eyes — 

Typically a combination of upper eyelid dermatochalasis and some degree of brow descent, rather than one single cause. This is why hooding can look different from person to person even when the underlying skin changes are similar.

Crepey texture — 

Thinning skin and reduced elastin, distinct from any of the above. There’s an important distinction between two related terms here: thinning skin describes the underlying structural change — a loss of collagen and thickness that also makes skin more fragile and prone to bruising — while crepey texture describes what that thinning looks like on the surface: fine wrinkling with a texture similar to crepe paper. Crepiness is the visible result of thinning, not a separate condition. This shows up both upper and lower — a client might have crepey texture on its own, or layered on top of hooding or excess skin, which is a common combination rather than an exception.

Stretched skin from previous filler —

Not everyone’s under-eye excess skin is age-related. Overfilled dermal filler under the eyes can stretch the skin over time, creating excess and laxity that has nothing to do with how old you are. This is a distinct cause on its own, since the usual “aging” explanations don’t apply here — and it’s one Plexr and NeoGen can address directly, since the underlying issue is the same excess skin, regardless of what caused it.

Dark circles —

Dark circles generally fall into three types, and it’s common to have more than one at the same time.

Vascular dark circles occur when underlying vessels and tissue show through the very thin skin beneath the eyes, creating a blue, purple or reddish appearance. They may become more noticeable with things like fatigue, alcohol or congestion.

Pigmented dark circles come from increased melanin and may be hereditary, sun-related or inflammation-driven. Allergies and eye rubbing can contribute to either type: congestion can impair venous drainage and create the vascular appearance often called “allergic shiners,” while repeated rubbing and irritation can contribute to post-inflammatory pigmentation [covered in more detail here].

Structural dark circles are different again. Tear-trough hollowing, age-related volume loss, skin laxity, thinning tissue or facial anatomy can create shadows beneath the eye that make the area appear darker. Genetics can contribute to all three types.

Which of these is happening changes what helps. Treatments like Plexr and NeoGen that improve collagen, skin quality and structural support are most relevant when thin, lax or hollow tissue is contributing to the darkness. They don’t directly remove melanin pigmentation or correct an underlying vascular cause, although improving thin under-eye skin may make underlying vessels less visible. Hooded Eyes

The difference is permanence: Botox achieves this temporarily by relaxing muscle, while Plexr’s effect on the treated area doesn’t fade the way Botox does.

Eye Bags vs. Hooded Eyes — Why They're Not the Same Problem

Eye bags happen lower on the eye and can come from a few different things, not just one. Fat pushing forward through a weakened membrane is the classic cause, but factors such as skin laxity, fluid retention, and allergies can all contribute too, sometimes on their own and sometimes together. Hooding happens upper on the eye and is primarily a skin-and-brow issue. Both can exist on the same face without being related to each other. Droopy or Saggy Eyelids — When It's Asymmetry, When It's Just Aging Skin

If one eyelid looks noticeably different from the other, that deserves a closer look — asymmetric changes are more often related to true ptosis (the muscle) than to dermatochalasis (the skin), and that distinction matters for what will help. A simple way to tell the difference at home: gently lift the excess skin fold up toward your brow. If the eyelid underneath looks essentially normal once the skin is out of the way, that points toward dermatochalasis. If the lid margin itself still sits low even with the skin lifted, that points toward ptosis.

If what you’re noticing points toward true ptosis or a real asymmetry between the two sides, that’s worth a conversation with your physician before choosing a treatment path. Plexr is excellent for excess skin and laxity, but it doesn’t correct the muscle itself — for true ptosis, surgical correction is often the more effective option, and sometimes the only one that will give the result you’re actually looking for. Part of a good consultation is being honest about which category you fall into, not fitting you into whatever a practitioner happens to offer.

Non-Surgical vs. Surgical Blepharoplasty — Setting Honest Expectations

Surgical blepharoplasty removes excess skin through incision and, when needed, repositions fat or repairs the levator muscle directly — it remains the definitive answer for true ptosis affecting vision, or for skin excess severe enough to obstruct the visual field.

“Non-surgical” doesn’t mean nothing is removed — it means there’s no cutting. Plexr is non-surgical, but it does remove tissue: sublimation converts the outermost layer of skin directly, without a blade or incision, triggering the same kind of contraction a surgical removal would, just without surgery. Yes — overfilled dermal filler can stretch the skin over time, creating excess and laxity that isn’t related to aging at all. Plexr and NeoGen can typically address this well because the treatment target is the excess skin, regardless of what caused it.

A simple clue is the eyelid stretch test. Gently stretch the lower-eyelid skin outward. If the colour remains, pigmentation is more likely. With a vascular cause, the colour may lighten or the underlying vessels may become more apparent.

Structural darkness behaves more like a shadow. It tends to change with lighting, angle and the contour beneath the eye rather than behaving like fixed skin pigment.

It’s not a perfect at-home test — especially because many dark circles are a combination of vascular, pigmented and structural causes — but it can give you a useful clue about what’s contributing.

Most often, it’s related to the natural decline in collagen and elastin with age, though hormonal changes — including those in perimenopause and menopause — can accelerate it.
